STXSA Clinician Contract: This contract should be filled in by a member of the event committee and given to the clinician to complete and return along with the W-9 form. Both forms should be returned to the STXSA treasurer.
W-9 Request for Taxpayer ID: This form should be given to a clinician with the Clinician Contract.
Check Request Form: This form is to be submitted to the STXSA treasurer to request a check
a) for reimbursement,
b) to pay an individual for services (clinicians, accompanists, etc.),
c) to pay a facility or company for goods.
Income Receipt Form: This form is to be submitted to the STXSA treasurer with income (cash and checks) collected for an event including entry fees, donations and other income.
Chairmans Financial Report STXSA: This form is for teachers to fill out after an event. Please attach any receipts or copies of receipts to the STXSA treasurer.
Student Scholarship App: This is the application form for a student to apply for an STXSA student scholarship
Teacher Recommendation Form: The teacher of a student applying for an STXSA scholarship must fill out this form.
